Player Release

YOUTH SYB BASEBALL PARENTAL CONSENT, RELEASE OF LIABILITY AND INDEMNIFICATION AGREEMENT


As the parent or legal guardian of the minor child or ward (“Participant”) participating in Stow Youth Baseball (“SYB”), I give my full consent and approval for my Participant to participate in any and all activity related to Baseball. 

 

I am signing that I understand that the very nature of the SYB Baseball is potentially hazardous, due to, but not limited to, pitching the ball, swinging a bat, hitting the ball, fielding the ball, running, jumping, swinging the bat, warm-up exercises, sliding, diving and collisions with other players, volunteers or stationary objects. Inherent hazards also include personal contact and interaction that could facilitate the spread of COVID-19. Sports are not immune from COVID-19, and provide an environment for potential spread of this extremely contagious virus. As an extremely contagious virus, I understand that my Participant will be interacting with other players, coaches, teams, instructors, participants, umpires, volunteers, suppliers, vendors, equipment and facilities (“Releasees”) that could spread COVID-19, or any and all other seasonal colds, flu and viruses. All of the actions and conditions described, and others mentioned and not mentioned, could cause injury or sickness, including death, to my Participant and to others. I consent that I understand these risks and am willing to assume these risks on behalf of my Participant and to the following:

 

  1. I do voluntarily elect, on behalf of myself and my Participant, to accept and assume all liability and risks of injury and sickness incurred by my Participant before, after and while attending SYB Baseball and related events and while on the SYB premises or any and all other related premises.
  2. I certify that my Participant is fully capable of participating in Baseball and that my Participant is healthy and has no underlying conditions, disabilities or infirmities that would restrict full participation in these activities. 
  3. I agree on behalf of myself and my Participant listed below, that I will hold harmless and fully indemnify Stow Youth Baseball and Releasees associated Baseball and that the Releasees are hereby released from any and all loss, claims, expense, injury, damages, and costs which may arise from any cause of action made by me or by, through or on behalf of my Participant, even if the damages are caused in whole or in part by any of the Releasees herby released.

 

I acknowledge that I have read each and every one of the provisions in this Parental Consent Form, Release of Liability and Indemnification Agreement, that I understand each of the provisions in this agreement and that I agree to abide by them. This Agreement shall be in effect and binding upon Participant’s heirs, next of kin, executors, assigns and representatives.
